Both and "it depends." Essentially, pixel perfect websites are developed to match a design mock-up (pretty much the entire thing is custom web development with code). Whereas, bootstrap websites utilizing themes (templates and modules) that are pre-developed that users can drop & drop onto a page–without needing to code anything–to create layouts without a developer.
